Identification Requirements

All applicants must submit a copy of an accepted form of identification and a copy of proof of relationship (if required). After placing your order, you will need to upload an image of your identification and proof of relationship (if required). Your order will only be processed once a legible copy of identification and proof of relationship (if required) are received along with payment.

All applicants must provide a valid photo identification as required.

Valid Photo Identification: Submit a copy of 1 of the following:

- Driver's License
- State ID Card

If you cannot provide any from the above list, you must submit at least 2 of the following with your name on them. In addition to your name, one piece must also have your current address:

- One may be a bill or other USPS mail

The second must be one of the following items:
- Medical/car insurance card
- credit card statement
- Paystub with imprinted information
- Voter registration card
- Car registration paperowrk
- Bank Statement
- Public assistance card
- Active duty military ID w/ issued and expiration dates
- EBT Link card (Illinois Electronic Benefit Transfer)
