Identification Requirements

All applicants must submit a copy of an accepted form of identification and a copy of proof of relationship (if required). After placing your order, you will need to upload an image of your identification and proof of relationship (if required). Your order will only be processed once a legible copy of identification and proof of relationship (if required) are received along with payment.

All applicants must provide a valid photo identification as required.

Valid Photo Identification: Submit a copy of 1 of the following:

- Alabama Driver's license
- Out-of-State Driver's License
- State Issued Non-Driver ID Card
- U.S. or Foreign Passport
- U.S Certificate of Naturalization
- Certificate of Citizenship
- U.S. Military ID
- Work ID (If applicant is employee of agency/company making request)
- Alien Resident Card (Temporary or Permanent)
- U.S. Employment Authorization Card
- Citizenship ID Card
- Tribal ID
- Pilot's License
- Boating License
- Concealed Weapons License
- Ex-Felon ID
- Inmate ID issued by the U.S. Dept. of Justice w/ following documentation: Supporting documents from intuition if inmate is still in custody or letter of release from institution in inmate has been released
- School ID (Must include current school term)
- Alabama Voter ID Card

If you cannot provide any from the above list, you must submit at least 2 of the following:

- Expired, Government-Issed ID
